Multiple errors led to fuel leak in Pearl Harbor water, Navy says | Navy Times

Fuel from a tank farm from World War II flowed into a well that provided drinking water to homes and workplaces in Hawaii. Approximately 6,000 individuals, including members of the military, experienced nausea, headaches, rashes, and other symptoms. The investigation report detailed a cascade sequence of errors commencing in May 2021. The earliest date the Department of Defense could safely defuel the tanks is December 2024. Concerns that the oil may move down the aquifer and into the city’s wells led the Honolulu Board of Water Supply to shut a crucial well servicing around 400,000 residents. According to Hawaii’s Department of Health, Red Hill must be shut down as soon as possible. According to a study, inadequate training and supervision, inefficient leadership, and a lack of accountability led to the occurrence. A lawyer for Earthjustice, which has filed legal challenges against the facility, asserts that the Navy has not learned from its past blunders.
