The F-15C fighter jet crashed off the coast of England earlier this year that killed Air Force 1st Lt. Kenneth “Kage” Allen. The plane plunged into the North Sea and investigation ruled out aircraft malfunction, component failure, and maintenance issues as contributing factors to the crash. According to the Air Force accident investigation board, it was the result of the pilot’s fixation on the intercept of the simulated adversary aircraft and failure to execute cockpit instrument visual scans while flying through cloud cover and experiencing spatial disorientation.
